Student Physicals:

MSHSAA policy allows for sports physicals to be valid for 2 years, per physician approval.  However, there are also annual requirements that need to be returned to the school in order for a student-athlete to participate.  Therefore, there are 2 separate forms that must be filled out for a valid sports physical.  Part 1 is the physician evaluation form that can be good for up to 2 years per physician's approval.  Part 2 are the annual requirements that must be updated every year.  These forms must be completed and kept on file with the school before a student-athlete may participate in MSHSAA sanctioned activities.  Thank you
